Outreach Services
Key Responsibilities:
- Coordinate with patients and their attendants to schedule appointments for home health visits.
- Assist patients and families by providing relevant information, responding to queries, and addressing concerns.
- Arrange and manage home health visits as per the approved schedule.
- Liaise with doctors, nurses, and ward coordinators for home health referrals generated within the hospital for post-discharge services.
- Follow up with patients during their hospital stay and after discharge, and coordinate their home care management plan based on their specific needs.
- Maintain and update patient records and home visit data in a timely and accurate manner.
- Promote home health services within wards and clinics, and facilitate patient registration for home-based care.
- Collaborate with the Marketing Department for awareness and promotion of home health services.
Qualifications and Experience:
- Candidate should hold at least a Master’s degree with at least 2-4 years of related work experience.
- Proficiency in the use of MS Office applications.
- Good communication and interpersonal skills
